A slide shown by the U.S. State Department at the AIDS 2026 conference in Rio de Janeiro, attended by more than 20,000 people from 190 countries, displayed a map of Africa with every featured country in the wrong location — Nigeria appeared landlocked in the Sahara, Mozambique was moved to the Horn of Africa, and Cameroon was missing entirely. A Reuters analysis found the image carried a watermark indicating it was generated with OpenAI tools. The State Department took responsibility, saying a staff member had hastily edited the slide deck shortly before the presentation without adequate review.
