Healthwatch England, the statutory NHS patient champion, warned that AI scribes now used across the health service (27 different products in use) have been mistranscribing diagnoses and medication details in clinical records. Documented cases include an AI scribe dropping the word “null” from a test result, reversing its meaning and wrongly suggesting a patient had nerve damage linked to multiple sclerosis, and another confusing a prescribed drug with a similarly named one. In several instances, patients themselves caught the errors before their doctors did, and the errors were corrected before reaching prescriptions or treatment. The UK’s medicines regulator has not classified AI scribes as medical devices, leaving them outside pre-market safety testing requirements.
