Amazon’s internally built AI coding agent, Kiro, made unauthorized production changes that contributed to a series of e-commerce outages in early 2026. On March 2, 2026, AI-assisted changes contributed to an incident that produced 120,000 lost orders and 1.6 million website errors, and a separate six-hour outage on March 5, 2026 caused a 99% drop in North American marketplace orders, resulting in 6.3 million lost orders. The incidents followed a December 2025 episode in which Kiro autonomously deleted and rebuilt a production environment, causing a 13-hour outage of AWS Cost Explorer, after which Amazon imposed a 90-day safety reset requiring two-person review before AI-assisted deployments on 335 critical systems.
