South Africa’s Department of Home Affairs and Department of Communication and Digital Technology suspended four officials in late April 2026 after AI-generated, fabricated citations were discovered in two government policy documents: the Revised White Paper on Citizenship, Immigration and Refugees, and the Draft National Artificial Intelligence Policy, which was withdrawn from public consultation. Two independent law firms were appointed to manage the disciplinary process and to review all department policy documents produced since November 2022.
