A Tesla Model 3 operating with Full Self-Driving engaged left a residential road in Katy, Texas, and struck a house at roughly 73 mph, killing 76-year-old Martha Avila Mantilla, who was standing inside. Investigators found no mechanical failure and no evidence of a medical episode or impairment in the driver, who had researched complaints that FSD was “too timid” in the weeks before the crash. Tesla says the driver overrode the system by flooring the accelerator; the driver, Michael David Butler, was charged with manslaughter on July 1, 2026. The National Highway Traffic Safety Administration has opened a federal investigation into the crash.
